The
aging process affects the entire family, often creating a strain
on their emotional, psychological and financial resources. As
a geriatric care manager, I work with you to help determine
the needs of your elderly person and establish a course of action
tailored to those specific needs. The goal is to maintain and
enhance the well-being, independence and dignity of the older
person while being sensitive to family needs and resources. |

care manager is an advanced degreed professional who specializes
in working with older people and their families to identify
and attend to multiple needs of the aging process as it affects
a specific individual. |

The
process starts with a thorough assessment, in the home, of the
current living conditions, strengths, and limitations of the
older person. Then contacts are made with important people such
as the doctor, and any other service providers. Finally, a carefully
designed plan is provided, to address all facets of their living.
This is reviewed with the client and/or family and the implementation
is agreed on. |

your care manager, I am also a licensed clinical social worker,
and therefore, subject to the National Association of Social
Workers Code of Ethics and the State of Florida Regulations
for Social Workers.
Her many years of work with the disabled, elderly and terminally
ill, has given her a thorough grounding in the special needs
